Standard Form for Endnotes & Bibliographies
Working with footnotes in today's desktop publishing programs is prohibitively complicated. Hopefully, this problem will be addressed and solved soon. In the meantime, Back Channel Press will only accept manuscripts employing endnotes.

Ibid. (the abbreviation for the Latin Ibidem, meaning "the same") refers to the same title as the last endnote just cited before.
